PG Degree/Diploma certificate.

DMC registration certificate with MD/MS/DNB qualification.

NOC from the present employer (If working in any other company).

Experience certificate (If required)

Income and asset certificate.

How to pay Fees?

Candidates can make the payment for their fees through Demand Draft in favour of "ESIC Saving Fund Account No.2" Payable at "New Delhi".
